OnCoCo 1.0: A Public Dataset for Fine-Grained Message Classification in Online Counseling Conversations

This paper presents OnCoCo 1.0, a new public dataset for fine-grained message classification in online counseling. It is based on a new, integrative system of categories, designed to improve the automated analysis of psychosocial online counseling conversations. Existing category systems, predominantly based on Motivational Interviewing (MI), are limited by their narrow focus and dependence on datasets derived mainly from face-to-face counseling. This limits the detailed examination of textual counseling conversations. In response, we developed a comprehensive new coding scheme that differentiates between 38 types of counselor and 28 types of client utterances, and created a labeled dataset consisting of about 2.800 messages from counseling conversations. We fine-tuned several models on our dataset to demonstrate its applicability. The data and models are publicly available to researchers and practitioners. Thus, our work contributes a new type of fine-grained conversational resource to the language resources community, extending existing datasets for social and mental-health dialogue analysis.


CARE-Bench: A Benchmark of Diverse Client Simulations Guided by Expert Principles for Evaluating LLMs in Psychological Counseling

The mismatch between the growing demand for psychological counseling and the limited availability of services has motivated research into the application of Large Language Models (LLMs) in this domain. Consequently, there is a need for a robust and unified benchmark to assess the counseling competence of various LLMs. Existing works, however, are limited by unprofessional client simulation, static question-and-answer evaluation formats, and unidimensional metrics. These limitations hinder their effectiveness in assessing a model's comprehensive ability to handle diverse and complex clients. To address this gap, we introduce \textbf{CARE-Bench}, a dynamic and interactive automated benchmark. It is built upon diverse client profiles derived from real-world counseling cases and simulated according to expert guidelines. CARE-Bench provides a multidimensional performance evaluation grounded in established psychological scales. Using CARE-Bench, we evaluate several general-purpose LLMs and specialized counseling models, revealing their current limitations. In collaboration with psychologists, we conduct a detailed analysis of the reasons for LLMs' failures when interacting with clients of different types, which provides directions for developing more comprehensive, universal, and effective counseling models.


Beyond Empathy: Integrating Diagnostic and Therapeutic Reasoning with Large Language Models for Mental Health Counseling

Large language models (LLMs) hold significant potential for mental health support, capable of generating empathetic responses and simulating therapeutic conversations. However, existing LLM-based approaches often lack the clinical grounding necessary for real-world psychological counseling, particularly in explicit diagnostic reasoning aligned with standards like the DSM/ICD and incorporating diverse therapeutic modalities beyond basic empathy or single strategies. To address these critical limitations, we propose PsyLLM, the first large language model designed to systematically integrate both diagnostic and therapeutic reasoning for mental health counseling. To develop PsyLLM, we design a novel automated data synthesis pipeline that processes real-world mental health posts collected from Reddit, where users frequently share psychological distress and seek community support. This pipeline processes real-world mental health posts, generates multi-turn dialogue structures, and leverages LLMs guided by international diagnostic standards (e.g., DSM/ICD) and multiple therapeutic frameworks (e.g., CBT, ACT, psychodynamic) to simulate detailed clinical reasoning processes. Rigorous multi-dimensional filtering ensures the generation of high-quality, clinically aligned dialogue data. In addition, we introduce a new benchmark and evaluation protocol, assessing counseling quality across four key dimensions. Our experiments demonstrate that PsyLLM significantly outperforms state-of-the-art baseline models on this benchmark. TheraMind: A Strategic and Adaptive Agent for Longitudinal Psychological Counseling

Large language models (LLMs) in psychological counseling have attracted increasing attention. However, existing approaches often lack emotional understanding, adaptive strategies, and the use of therapeutic methods across multiple sessions with long-term memory, leaving them far from real clinical practice. To address these critical gaps, we introduce TheraMind, a strategic and adaptive agent for longitudinal psychological counseling. The cornerstone of TheraMind is a novel dual-loop architecture that decouples the complex counseling process into an Intra-Session Loop for tactical dialogue management and a Cross-Session Loop for strategic therapeutic planning. The Intra-Session Loop perceives the patient's emotional state to dynamically select response strategies while leveraging cross-session memory to ensure continuity. Crucially, the Cross-Session Loop empowers the agent with long-term adaptability by evaluating the efficacy of the applied therapy after each session and adjusting the method for subsequent interactions. We validate our approach in a high-fidelity simulation environment grounded in real clinical cases. Extensive evaluations show that TheraMind outperforms other methods, especially on multi-session metrics like Coherence, Flexibility, and Therapeutic Attunement, validating the effectiveness of its dual-loop design in emulating strategic, adaptive, and longitudinal therapeutic behavior. HealthDial: A No-Code LLM-Assisted Dialogue Authoring Tool for Healthcare Virtual Agents

We introduce HealthDial, a dialogue authoring tool that helps healthcare providers and educators create virtual agents that deliver health education and counseling to patients over multiple conversations. HealthDial leverages large language models (LLMs) to automatically create an initial session-based plan and conversations for each session using text-based patient health education materials as input. Authored dialogue is output in the form of finite state machines for virtual agent delivery so that all content can be validated and no unsafe advice is provided resulting from LLM hallucinations. LLM-drafted dialogue structure and language can be edited by the author in a no-code user interface to ensure validity and optimize clarity and impact. We conducted a feasibility and usability study with counselors and students to test our approach with an authoring task for cancer screening education. Participants used HealthDial and then tested their resulting dialogue by interacting with a 3D-animated virtual agent delivering the dialogue. Through participants' evaluations of the task experience and final dialogues, we show that HealthDial provides a promising first step for counselors to ensure full coverage of their health education materials, while creating understandable and actionable virtual agent dialogue with patients.


MIRROR: Multimodal Cognitive Reframing Therapy for Rolling with Resistance

Recent studies have explored the use of large language models (LLMs) in psychotherapy; however, text-based cognitive behavioral therapy (CBT) models often struggle with client resistance, which can weaken therapeutic alliance. To address this, we propose a multimodal approach that incorporates nonverbal cues, which allows the AI therapist to better align its responses with the client's negative emotional state. Specifically, we introduce a new synthetic dataset, Mirror (Multimodal Interactive Rolling with Resistance), which is a novel synthetic dataset that pairs each client's statements with corresponding facial images. Using this dataset, we train baseline vision language models (VLMs) so that they can analyze facial cues, infer emotions, and generate empathetic responses to effectively manage client resistance. These models are then evaluated in terms of both their counseling skills as a therapist, and the strength of therapeutic alliance in the presence of client resistance. Our results demonstrate that Mirror significantly enhances the AI therapist's ability to handle resistance, which outperforms existing text-based CBT approaches. Human expert evaluations further confirm the effectiveness of our approach in managing client resistance and fostering therapeutic alliance.


AnnaAgent: Dynamic Evolution Agent System with Multi-Session Memory for Realistic Seeker Simulation

Constrained by the cost and ethical concerns of involving real seekers in AI-driven mental health, researchers develop LLM-based conversational agents (CAs) with tailored configurations, such as profiles, symptoms, and scenarios, to simulate seekers. While these efforts advance AI in mental health, achieving more realistic seeker simulation remains hindered by two key challenges: dynamic evolution and multi-session memory. Seekers' mental states often fluctuate during counseling, which typically spans multiple sessions. To address this, we propose AnnaAgent, an emotional and cognitive dynamic agent system equipped with tertiary memory. AnnaAgent incorporates an emotion modulator and a complaint elicitor trained on real counseling dialogues, enabling dynamic control of the simulator's configurations. Additionally, its tertiary memory mechanism effectively integrates short-term and long-term memory across sessions. Evaluation results, both automated and manual, demonstrate that AnnaAgent achieves more realistic seeker simulation in psychological counseling compared to existing baselines. DiaCBT: A Long-Periodic Dialogue Corpus Guided by Cognitive Conceptualization Diagram for CBT-based Psychological Counseling

Psychotherapy reaches only a small fraction of individuals suffering from mental disorders due to social stigma and the limited availability of therapists. Large language models (LLMs), when equipped with professional psychotherapeutic skills, offer a promising solution to expand access to mental health services. However, the lack of psychological conversation datasets presents significant challenges in developing effective psychotherapy-guided conversational agents. In this paper, we construct a long-periodic dialogue corpus for counseling based on cognitive behavioral therapy (CBT). Our curated dataset includes multiple sessions for each counseling and incorporates cognitive conceptualization diagrams (CCDs) to guide client simulation across diverse scenarios. To evaluate the utility of our dataset, we train an in-depth counseling model and present a comprehensive evaluation framework to benchmark it against established psychological criteria for CBT-based counseling. Results demonstrate that DiaCBT effectively enhances LLMs' ability to emulate psychologists with CBT expertise, underscoring its potential for training more professional counseling agents.

With the rapid development of digital technology, AI-driven psychological counseling has gradually become an important research direction in the field of mental health. However, existing models still have deficiencies in dialogue safety, detailed scenario handling, and lightweight deployment. To address these issues, this study proposes PsyLite, a lightweight psychological counseling large language model agent developed based on the base model InternLM2.5-7B-chat. Through a two-stage training strategy (hybrid distillation data fine-tuning and ORPO preference optimization), PsyLite enhances the model's deep-reasoning ability, psychological counseling ability, and safe dialogue ability. After deployment using Ollama and Open WebUI, a custom workflow is created with Pipelines. An innovative conditional RAG is designed to introduce crosstalk humor elements at appropriate times during psychological counseling to enhance user experience and decline dangerous requests to strengthen dialogue safety. Evaluations show that PsyLite outperforms the baseline models in the Chinese general evaluation (CEval), psychological counseling professional evaluation (CPsyCounE), and dialogue safety evaluation (SafeDialBench), particularly in psychological counseling professionalism (CPsyCounE score improvement of 47.6\%) and dialogue safety (\safe{} score improvement of 2.4\%). Additionally, the model uses quantization technology (GGUF q4\_k\_m) to achieve low hardware deployment (5GB memory is sufficient for operation), providing a feasible solution for psychological counseling applications in resource-constrained environments.


EmoStage: A Framework for Accurate Empathetic Response Generation via Perspective-Taking and Phase Recognition

The rising demand for mental health care has fueled interest in AI-driven counseling systems. While large language models (LLMs) offer significant potential, current approaches face challenges, including limited understanding of clients' psychological states and counseling stages, reliance on high-quality training data, and privacy concerns associated with commercial deployment. To address these issues, we propose EmoStage, a framework that enhances empathetic response generation by leveraging the inference capabilities of open-source LLMs without additional training data. Our framework introduces perspective-taking to infer clients' psychological states and support needs, enabling the generation of emotionally resonant responses. In addition, phase recognition is incorporated to ensure alignment with the counseling process and to prevent contextually inappropriate or inopportune responses. Experiments conducted in both Japanese and Chinese counseling settings demonstrate that EmoStage improves the quality of responses generated by base models and performs competitively with data-driven methods.
