Navigating Fairness in Radiology AI: Concepts, Consequences,and Crucial Considerations

Artificial Intelligence (AI) has significantly revolutionized radiology, promising improved patient outcomes and streamlined processes. However, it's critical to ensure the fairness of AI models to prevent stealthy bias and disparities from leading to unequal outcomes. This review discusses the concept of fairness in AI, focusing on bias auditing using the Aequitas toolkit, and its real-world implications in radiology, particularly in disease screening scenarios. Aequitas, an open-source bias audit toolkit, scrutinizes AI models' decisions, identifying hidden biases that may result in disparities across different demographic groups and imaging equipment brands. This toolkit operates on statistical theories, analyzing a large dataset to reveal a model's fairness. It excels in its versatility to handle various variables simultaneously, especially in a field as diverse as radiology. The review explicates essential fairness metrics: Equal and Proportional Parity, False Positive Rate Parity, False Discovery Rate Parity, False Negative Rate Parity, and False Omission Rate Parity. Each metric serves unique purposes and offers different insights. We present hypothetical scenarios to demonstrate their relevance in disease screening settings, and how disparities can lead to significant real-world impacts.


Automatic Fairness Testing of Neural Classifiers through Adversarial Sampling

Although deep learning has demonstrated astonishing performance in many applications, there are still concerns about its dependability. One desirable property of deep learning applications with societal impact is fairness (i.e., non-discrimination). Unfortunately, discrimination might be intrinsically embedded into the models due to the discrimination in the training data. As a countermeasure, fairness testing systemically identifies discriminatory samples, which can be used to retrain the model and improve the model's fairness. Existing fairness testing approaches however have two major limitations. Firstly, they only work well on traditional machine learning models and have poor performance (e.g., effectiveness and efficiency) on deep learning models. Secondly, they only work on simple structured (e.g., tabular) data and are not applicable for domains such as text. In this work, we bridge the gap by proposing a scalable and effective approach for systematically searching for discriminatory samples while extending existing fairness testing approaches to address a more challenging domain, i.e., text classification. Compared with state-of-the-art methods, our approach only employs lightweight procedures like gradient computation and clustering, which is significantly more scalable and effective. Experimental results show that on average, our approach explores the search space much more effectively (9.62 and 2.38 times more than the state-of-the-art methods respectively on tabular and text datasets) and generates much more discriminatory samples (24.95 and 2.68 times) within a same reasonable time. Moreover, the retrained models reduce discrimination by 57.2% and 60.2% respectively on average.

In 2018, new hardware developments allowed more sophisticated and widespread use of AI, and software managed to make AI even more human-like. Take a look back at the developments in AI over the past year, and a look ahead to what may come next. The concept of artificial intelligence has been around for centuries, such as the story of the automaton Talos from ancient Greek mythology. But in 2018, as AI seeped further into everyday life, the rapid developments in AI hardware and applications sparked debate as to its very existence. Depending on how we define both "artificial" and "intelligence", we can determine whether we see AI as a marketing myth, a legitimate stride in technology, or a harbinger of a terrifying future.


Aequitas: A Bias and Fairness Audit Toolkit

Recent work has raised concerns on the risk of unintended bias in algorithmic decision making systems being used nowadays that can affect individuals unfairly based on race, gender or religion, among other possible characteristics. While a lot of bias metrics and fairness definitions have been proposed in recent years, there is no consensus on which metric/definition should be used and there are very few available resources to operationalize them. Therefore, despite recent awareness, auditing for bias and fairness when developing and deploying algorithmic decision making systems is not yet a standard practice. We present Aequitas, an open source bias and fairness audit toolkit that is an intuitive and easy to use addition to the machine learning workflow, enabling users to seamlessly test models for several bias and fairness metrics in relation to multiple population sub-groups. We believe Aequitas will facilitate informed and equitable decisions around developing and deploying algorithmic decision making systems for both data scientists, machine learning researchers and policymakers.
